Job Type: Full Time – Permanent
Location: Airway Heights, WA
Schedule: 7:00am-3:30pm Monday-Friday
Monthly Salary Range: $10,723.00-$13,061.00
Yearly Salary Range: $128,676.00-$156,732.00

The Department of Corrections is seeking a highly motivated and qualified individual for the critical role of Clinical Pharmacist for the Airway Heights Corrections Center located in Airway Heights, WA. This is a full-time day shift position with Sat/Sun off.

The Clinical Pharmacist supports the agencies mission by enhancing patient health and wellness via evidence-based pharmaceutical care directly to DOC patients receiving clinical services in DOC facilities and in preparation for reentry into the community. The primary objective of this position is to apply these evidence-based practices to better optimize therapeutic outcomes, provide cost-effective pharmacotherapy and educate staff and patients on proper medication utilization; therefore, focusing on best practice patient centered care.

Duties
This position is directly responsible for managing pharmaceutical resources in accordance with DOC Pharmacy and Therapeutics (P&T) Committee direction, DOC Health Services Policy, and Federal & State Law.

As the Clinical Pharmacist, duties include:

Working as an active member of a multidisciplinary team and collaborates with healthcare providers within facility patient care areas and across the State to provide evidence-based, patient-centered pharmaceutical clinical care:

Maintains proficiency in DOC Health Plan provided services and knowledge necessary to provide safe and effective medication therapy, including evidence-based medication treatment, disease management, and legal and regulatory requirements related to pharmacy.
Collaborates with the clinical team to individualize drug therapies promoting safe, effective, and economical patient care.
Completes patient assessments, monitoring, and reviews patient health
records/profiles to identify, prevent or mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication and treatment failures.
Makes appropriate evidence-based, patient-centered medication recommendations. Provides comprehensive written and/or oral medication information and patient consultation communication to other healthcare professionals, including literature search and critical evaluation of the involved topic.
Utilizes the patient health record and other appropriate documentation systems to communicate with other healthcare professionals and to document specific pharmacotherapeutic recommendations.
6Works under approved collaborative drug therapy agreements (CDTA) to initiate or adjust therapy to include the ordering and interpretation of laboratory testing.
Participates in clinic/infirmary rounds and/or conferences with other healthcare professionals. Assesses medication therapy and recommends management solutions as appropriate.
Actively leads and/or participates in stewardship activities and medication monitoring/management programs such as anticoagulation management, pain management, diabetes care, oncology, antibiotic stewardship, etc.
Provides patient counseling and education.
Assists the Director of Pharmacy with management of pharmacy related clinical and operational programs:

Participates in review of Non-Formulary Requests.
Serves as a pharmacist member of the Pharmacy and Therapeutics (P&T) Committee; attends all scheduled meetings.
Reviews assigned Formulary sections as assigned for new safety, efficacy and/or cost information and presents to the P&T Committee.
Develops drug monographs for P&T Committee review of new drug classes or new drugs for addition to the Formulary.
Assists in the development of medication-specific protocols to guide treatment.
Provides Health Care Entity oversight of assigned facility as Responsible Pharmacy Manager per WAC 246-945-310.
Participates in DOC Coordinated Quality Improvement Program (CQIP) activities:

Coordinates with the central pharmacy, facility and statewide quality teams regarding medication therapy safety issues.
Identifies high-risk patients and implements interventions to improve quality and safety.
Participates in the Medication Incident Review process.
Actively attends local CQIP meetings.
Other duties as assigned:

Promotes safety; be an active participant, be aware, be alert, and make recommendations.
Attends all required training.
Remains current concerning DOC Health Services policies and procedures.

Qualifications
Required Qualifications:

A valid Washington State Pharmacist License.
AND

Two years of clinical consultative pharmacy expierence
Desired Qualifications:

Pharmacy residency desired.
Board of Pharmacy Specialties certification in Ambulatory Care, Geriatric Care, Infectious Disease, Oncology, Pharmacotherapy or Psychiatric Pharmacy desired.
Hospital, Long Term Care or Managed Care Pharmacy experience preferred.

Must be able to work in spaces, climb stairs, walk long distances, and frequently lift up to 30 pounds. Under rare circumstances this position may be required to lift or carry up to 50 pounds and push up to 100 pounds.

Salary is based on experience and is determined at the time a final offer of employment is made. The salary range listed in the job posting includes Steps A-M. Step M is a longevity step and is typically gained six years after being assigned to Step L in your permanent salary range.
